SWAPS is registered with CSCI as an adult placement scheme, delivering Shared Lives services across Devon & Cornwall. We offer accomodation with care and support in the family homes of our approved Shared Lives providersto adults who may have a learning disability, mental health needs, a physical disability or needs related to older age. This can be a long term arrangement or for regular short breaks.
Our Shared Lives providers have been comprehensively assessed to ensure that they can offer services that are safe and welcoming and that build on the strengths and gifts of each individual.
We offer support and training to our Shared Lives providers to enable them to fulfil their role to the best of their abilities. We monitor the well being of our Shared Lives providers and service users through regular home visits and contact. We consult with our Shared Lives providers and service users so we can improve the service for everyone
Our Shared Lives services aim to:
Enable people to live life to the full within a safe and enabling family and community setting
- Offer people support and encouragement in all aspects of their life
- Promote and develop people’s independence and daily living skills
- Help people with their every day care and health needs
- Help people realise their ambitions and potential
To achieve these aims we carefully match service users with our Shared Lives providers, taking into account lifestyles, location, hobbies and interests as well as ensuring we can meet people’s specific care needs.
